How to access neckline and shoulders lined wedding gown

When dealing with a bulky wedding dress or gown, and the shoulders or neckline need to be altered, what is the easiest method to access these areas?  the issue will be if the garment is waisted and the bodice is sealed off from the skirt.  Normally you could put your hand up into the garment and do the alteration.  But when the gown is waisted it can be difficult.
